As of the 4th of December it is necessary to request a duplicate copy of the work permit by special form

By Order №458 dated 14.10.2015, the Federal Migration Service of the Russian Federation has approved a special application form for the issuance of a duplicate copy of the work permit if the original work permit is lost or becomes damaged. Up until the 4th of December 2015, the employer or the  foreign employee could apply for registration of the duplicate  work permit to the relevant territorial office of the  FMS by any form.

In addition, a special application form has been created and approved in the event it is required to change data, including the applicant’s name, surname or details regarding the identity of the document holder.

These application forms are to be filled out by the foreigner and be signed.
